Raiders Affiliation Request Form

This form is to be used when a Raiders team is requesting to affiliate a Raiders player from another Raiders team. This form can only be filled out the affiliating Team Head Coach or Manager.

Prior to completing this form, please make sure you have followed GELC Regulation 8.01 and consulted with the parents/guardians of the player AND the head coach of the player's rostered team. Rostered team games take precedence.

When selecting a player, make sure you reference GELC Regulation 8 - Affiliation Rules for all rules regarding player affiliations.
